The Nurse Practitioner is an integral part of the clinical care team providing primary care services to the patients and families served by CMWC.

Responsibilities
  1. Provides comprehensive, high quality and cost-effective primary care services to a diverse population in a patient and family centered environment.
  2. Within the course of a primary care visit prescribe appropriate medications, order necessary screening and diagnostic tests, and confers and refers for specialty care, when necessary.
  3. Performs health assessments and physical examinations for adults and children, develops patient plans and works with patients/families to develop self-management goals.
  4. Maintains the safety and security of the patient/family’s Personal Health Information and is compliant with HIPAA standards.
  5. Ensures all proper documentation is present in the patient’s medical record.
  6. Submits billing codes that reflect diagnosis and the nature of the patient visit.
  7. Participates in Quality Improvement Initiatives and provides input on the development of policies and procedures which impact the health delivery service.
  8. Complies with all mandatory trainings.
  9. Perform other duties, tasks, projects as assigned to meet changing business and clinical conditions and priorities.

Qualifications
  1. Current license in the State of California as a Registered Nurse, Nurse Practitioner
  2. Specialty certification
  3. Current DEA license
  4. Current CPR certification as a healthcare provider
  5. Proficiency with a EHR
  6. Bilingual (English/Spanish) - Preferred
  7. Treat all patients and colleagues with dignity and respect.
  8. Ability to work with diverse populations

Education & Experience

  1. Graduate of an approved nursing program as a Nurse Practitioner.
  2. Two or more years of progressive, hands–on experience in a medical practice.
  3. Experience with medical software and electronic medical records.
  4. Immaculate record-keeping and organizational skills.
  5. Excellent multi-tasking and critical thinking skills.

Working Conditions

The physical demands described here are representative of those required for the position. Position requires sitting, walking, standing, bending, and twisting in the performance of daily activities. The position requires hand manipulation and repetitive hand movement and fine coordination in using a computer keyboard. The position requires near and far vision in reading reports and use of a computer. Acute hearing is required in supporting meetings and providing phone and in‐person customer service. The position occasionally requires lifting and/or moving objects up to 20 pounds.
